These Terms of Use govern your access to and use of sincerehomecareny.com (the “Site”), operated by Sincere Care Agency. By using the Site, you agree to these terms. If you do not agree, please do not use the Site.
Please read Sections 2 and 3 carefully. They explain that we are a private company rather than a government agency, and that we cannot determine or guarantee your eligibility for any benefit program.
1. Who we are and what we do
Sincere Care Agency is a private, New York State licensed home care services agency. We provide non-medical home care support, such as help with bathing and dressing, meal preparation, mobility assistance, medication reminders, light housekeeping, and companionship, in the five boroughs of New York City and in Nassau County.
2. We are not a government agency
Sincere Care Agency is not a government agency. We are not affiliated with, endorsed by, sponsored by, or acting on behalf of Medicaid, Medicare, the Centers for Medicare & Medicaid Services, the New York State Department of Health, the New York City Human Resources Administration, or any other federal, state, or local government body.
Nothing on this Site is an official government communication. Government programs are described here only as general background information. For official information about Medicaid in New York, visit health.ny.gov (opens in a new tab) or contact the appropriate agency directly.
3. No guarantee of eligibility, hours, or outcome
Eligibility for Medicaid and for Medicaid-funded home care, the outcome of any clinical assessment, the specific services a person is authorized to receive, and the number of care hours approved are determined by government agencies and by Medicaid managed care plans. They are not determined by Sincere Care Agency.
Accordingly:
- We do not decide who qualifies for any program or benefit.
- We cannot promise you will be found eligible for anything.
- We cannot promise any particular number of care hours.
- We cannot promise a particular caregiver, schedule, or start date.
- All services are subject to caregiver availability and to applicable authorizations.
Submitting a form on this Site does not enroll you in any program, create a plan of care, establish a service agreement, or constitute an application for any benefit.
4. Not medical advice
Information on this Site is general and educational. It is not medical advice, diagnosis, or treatment, and using the Site does not create a provider-patient or clinician-patient relationship. Always seek the advice of a physician or other qualified health professional with questions about a medical condition. Never disregard professional medical advice or delay seeking it because of something you read here.

5. Contacting us and your consent
If you submit the inquiry form, you confirm that you are at least 18 years old, that the phone number you provided belongs to you or that you are authorized to receive calls and texts at it, and that you agree we may contact you at that number, including with automated technology. Do not submit medical, diagnostic, or other sensitive health information through the Site’s web form. The form is not designed to receive it.
6. Accuracy of information you provide
You agree to provide accurate, current, and complete information and not to submit information about another person without that person’s knowledge and permission where permission is required.
7. Acceptable use
You agree not to:
- Use the Site for any unlawful purpose
- Submit false, misleading, or fraudulent information
- Submit automated, bulk, or spam form entries
- Attempt to gain unauthorized access to the Site or its systems
- Interfere with the Site’s operation, security, or availability
- Scrape, harvest, or copy Site content for commercial use without permission
- Impersonate any person or misrepresent your affiliation with anyone
8. Reviews and testimonials

This section limits warranties about the website. It does not limit any obligation we owe you regarding home care services actually delivered under a separate service agreement, and it does not waive any right you have under New York home care regulations.
13. Limitation of liability
To the fullest extent permitted by law, Sincere Care Agency and its officers, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of, or inability to use, the Site. Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you. Nothing here limits liability that cannot be limited by law.
14. Indemnification
You agree to indemnify and hold harmless Sincere Care Agency from claims, losses, and expenses arising from your misuse of the Site or your breach of these terms.
15. Governing law
These terms are governed by the laws of the State of New York, without regard to conflict-of-law rules. Any dispute relating to the Site will be brought in the state or federal courts located in New York.
16. Changes to these terms
We may revise these terms. The “Last updated” date above shows when they last changed. Continuing to use the Site after a change means you accept the revised terms.
